• Address Change Request - A member uses this form to change their name and/or home address with WCERS. This request cannot be processed without required supporting documentation and a signature.

• Sample EDRO Plans 1, 2, & 3 - This form is used as a sample in the event of divorce for a member of plan 1, 2, or 3 actively working.

• Sample EDRO Plans 4 - This form is used as a sample in the event of divorce for a member of plan 4 actively working. 

• Sample QDRO Deferred Compensation - This form is used as a sample in the event of divorce for a member of deferred compensation.
