• Address Change Request - A member uses this form to change their name and/or home address with WCERS. This request cannot be processed without required supporting documentation and a signature.

• Direct Deposit Application/Change Request - Please contact our office at 313-224-5890 to request a form

• Federal Tax Withholding Form (W4-P) - Please complete this form if you are requesting a change to your monthly federal income tax deductions

• State of Michigan Tax Withholding Form (MI W4-P) - Please complete this form if you are requesting a change to your monthly state of Michigan income tax deductions.

• Information Verification Form 2024  - This form is used to confirm members information.
   

• Beneficiary Designation Defined Benefit Plan Form - This form is used to designate a retirees last monies to a beneficiary

• Email Authorization Form  - A member uses this form to request that their direct deposit statements be sent to their email address.
